APPLICATIONS

To view or print the .PDF forms you will need Adobe's Acrobat Reader, version 4.0 or greater. This reader/viewer is free and you can download it from Adobe's website. Follow the step-by-step instructions provided by Adobe to install the Acrobat Reader and view these documents.

For your convenience, most (if not all) of these forms can be completed on your computer by using the Acrobat Reader. Use tab to move from one box to the other, or you can click directly on the box you're wanting to complete. Pulldown choices are available for selecting Magisterial Districts. For checkboxes, just click the square/circle and the checkmark appears. The date format is the long date (i.e., January 1, 2011).
